Consent to the processing of personal data

I, the confirmed (hereinafter referred to as "Data Subject"),

hereby grant to the company FYAS s.r.o., Litovelská 117/6, 779 00 Olomouc, ID number: 17960215, the company is registered in the commercial register kept at the Regional Court in Ostrava, section C, insert 91586 e-mail contact info@faceyoga-academy.eu express consent to the processing of my personal data, within the meaning of EU Regulation 2016/67 9 (General Data Protection Regulation, GDPR) and under the conditions below:

1. Personal data that will be processed:

• Name and surname

• Email

• Mobile phone

• IP address

2. The above-mentioned personal data will be processed by the Administrator for a period of 10 years, will not be transferred to a third party, will not be subject to automated decision-making, including profiling, and will be processed for the following purposes:

sending technical and business communications regarding the company's operations

3. The data subject is aware that consent can be withdrawn at any time, for example by sending a letter to the company's contact details or an e-mail to: info@selavis.cz, info@pegas-travel.cz.

4. The processing of personal data is carried out by the Administrator, but the following processors may also process personal data for the Administrator:

• software providers: Webnode, Anchoice, Ježek

• possibly other providers of processing software, services and applications, which the company does not currently use.

5. The data subject is aware that he has the right to:

• withdraw consent at any time;

• request information from the Administrator about what personal data it processes;

• request an explanation from the Administrator regarding the processing of personal data;

• request access to this data from the Administrator and have it updated or corrected;

• request the Administrator to delete this personal data;

• in case of doubt regarding compliance with obligations related to the processing of personal data, contact the Administrator or the Office for Personal Data Protection.

6. As part of this consent, I also confirm that I agree and confirm that I have familiarized myself with and agree to the Personal Data Protection Policy and the Terms and Conditions stated on this website.
